The line that is a target, not a range

Total cholesterol, HDL and triglycerides are reported against ranges. LDL cholesterol usually is not: it is reported against a target, and the target depends on your cardiovascular risk rather than on the laboratory that measured it.

This is why two people can have the same LDL, one flagged and one not, and both reports be correct. Someone with a previous heart attack and someone with no risk factors at all are being measured against different objectives, set by a physician rather than by an analyser.

If your report prints a range for LDL, read it as the laboratory’s default rather than as your target. The number that applies to you comes from the consultation, not the page.

Fasting is no longer the default

Since 2016, the European Atherosclerosis Society and the European Federation of Clinical Chemistry and Laboratory Medicine have jointly recommended measuring lipids without fasting as the routine approach.

The reasoning is the size of the difference. After an ordinary meal, the maximal average changes are about +26 mg/dL for triglycerides, −8 mg/dL for total cholesterol and −8 mg/dL for LDL, while HDL, apolipoprotein B and lipoprotein(a) are not affected at all. Non-fasting and fasting values predict cardiovascular disease equally well.

Fasting is reconsidered in one situation: when non-fasting triglycerides come back above 5 mmol/L, or 440 mg/dL. Some laboratories still ask for a fast, and their instruction governs your sample.

How the lines relate

Triglycerides are the line most affected by what you did yesterday: a meal raises them, and so does alcohol the night before. A single raised triglyceride value on a non-fasting sample is rarely the finding it appears to be.

HDL moves slowly and is not disturbed by a meal. It is also the line where smoking shows: in a study grouping people by smoking intensity, heavy smokers showed higher total cholesterol, triglycerides and LDL with lower HDL.

Non-HDL cholesterol, printed on many reports, is simply total cholesterol minus HDL. It is not an extra test, and it is one of the figures the European consensus lists with its own flagging threshold.

What a lipid panel does not tell you

It does not measure the state of your arteries. It measures substances that, over years, contribute to that state. A single panel is one point on a line that only becomes informative once you have several.

And it says nothing about the rest of your cardiovascular risk: blood pressure, smoking, diabetes, family history and age all carry more weight than any one lipid line, which is exactly why the LDL target is personal.
