Three families, not twelve verdicts
A complete blood count measures red cells, white cells and platelets. Everything else on the page describes those three: the indices such as MCV and MCH describe the red cells, and the differential splits the white cells into neutrophils, lymphocytes, monocytes, eosinophils and basophils.
So a report with twelve lines is not twelve findings. It is three questions with supporting detail, and the supporting detail is what makes each question answerable.
How the lines are meant to be read together
Haemoglobin with MCV is the classic pair. A low haemoglobin says there is anaemia; the MCV starts to say what kind. Small red cells point one way, large red cells another, and the ferritin on the same report usually settles it.
White cells are read as a total and as a split. A raised total with a neutrophil predominance suggests one thing, a raised total with lymphocytes another, and neither is a diagnosis on its own.
Platelets are read with their own index, the mean platelet volume, which describes the average size of the platelets rather than their number.
Why healthy people get flagged here
A CBC has many lines, and a reference range is built so that a small share of healthy people fall outside it on any given test. On a panel this long, at least one flag is more likely than none, in someone who is perfectly well.
Smoking is the most common explanation nobody mentions. Comparing healthy smokers with non-smokers, studies find higher white cell counts, higher haemoglobin and higher haematocrit among smokers, and the effect grows with how much someone smokes. A flagged white cell count in a smoker often has a simpler cause than infection.
A recent infection, a recent vaccination, hard exercise and pregnancy all move these lines too, which is why the date and the context matter as much as the number.
What to do with a flagged line
Ask how far outside, not whether outside. A platelet count of 145 against a floor of 150 and a platelet count of 40 print the same letter and mean entirely different things.
Then compare with your own previous count if you have one. A stable value slightly outside the range is a different object from the same value arriving for the first time.
A blood count is not read alone in practice: it is read next to the reason the test was ordered.
